Forms

What would school be without forms to fill in?

 

Instructor Assessment Forms

These are forms that your 3rd party instructor will fill in when you go to lessons or other activities. If you participate in music lessons, swimming, soccer, skiing, drama club, or anything else, please have your instructor fill in one of these forms each term to submit with your portfolio.

If the parent is the instructor, such as teaching your own child guitar, you may fill in one of these forms.

 

Self-Assessment Forms

These are forms your child may use to assess his or her own performance with things like oral reports, drama performance, etc.

 

Portfolio Forms

These forms include things like your PE log, Reading Log, book report forms, etc.
